I could have been the culprit. I turned off all MD HV's using the "Group" OFF before beam was restored after the log downtime yesterday (swing shift); then when beam came back OK, I simply did "HV ON" for the entire group. Did not realize that there something special for this tube.
So, if the particular tube was turned to off, but its setpoint was not set to zero, it could have been me.
